XVII - The Stars?

How do you unlock XVII - The Stars??

In The Binding of Isaac, XVII - The Stars? is unlocked by earning the Ultra Greedier completion mark as Tainted Isaac on hard mode.

Removes Isaac's oldest passive item (ignoring starting items) and creates 2 random item pedestals from the current room's item pool. Does nothing if Isaac has no items.
