How do you unlock Spirit Shackles?

In The Binding of Isaac, Spirit Shackles is unlocked by earning the The Beast completion mark as The Forgotten (Character).

Structured pathSource-backed requirements are mapped into trackable steps.

On taking fatal damage, instead of dying, Isaac's ghost is chained to his dead body, and he can continue to fight with 1/2 of a heart, returning to life after 10 seconds. After it activates, it must be recharged by gaining a Soul Heart or Black Heart from any source.
